Required Forms and Screenings
Three documents must be provided to ETTC prior to beginning classes.
Criminal Background Check
Each student must have a criminal background screening completed prior to starting the program. Please see the Sheriff Release Record on the next page, complete it, and go to the Knox County Sheriff’s department. They will complete the screening ($15) and provide the report to you.
TB Skin Test
Each student must provide ETTC with a clear TB skin test completed within the last 12 months. You can get a 1-step TB Skin test at the local health department or at a physician’s office of your preference. If the test results are positive, a clear Chest X-Ray (with lab report) is required. Bring a copy of the screening results back to ETTC prior starting program.
We must receive your transcript from one of the following prior to starting program. The transcript MUST be mailed (not hand-delivered) from the issuing entity in an official envelope.
- A high school or other government body, such as a county school board;
- An official equivalency transcript or GED scoresheet received directly from the issuing entity;
- An official military document indicating that the student completed high school such as an Enlisted Record Brief to be maintained in the student file at Elite Technical Training Center, Inc. facility.
